Causal inference techniques for trials of complex interventions

چکیده

A statistical framework based on potential outcomes has been developed in which causal effects can be unambiguously defined, and the assumptions needed for their estimation clearly stated. This has also led to the development of new statistical methods that are especially designed for making causal inferences from non-randomised exposures under transparent, less restrictive and more plausible assumptions. I will provide an overview of common research questions in trials of complex interventions (psychological therapies) that target causal effects other than the effects of random treatment offers and are not easily assessed using standard methods. A pertinent example where the effects of non-randomised exposures are of interest is the investigation of treatment effect modification by post-treatment characteristics of therapy such as therapeutic alliance. A second example relates to mechanism investigations in trials of complex interventions. The active components of a complex intervention typically target intermediate process variables in order to improve clinical outcome. Thus it is of interest to investigate empirically whether treatment effect mediation does indeed take place. I will show that such research questions require careful statistical design and analysis to enable consistent estimation of causal parameters of interest. In particular, statistical methods not typically used in routine analysis of clinical trials outcomes, such as instrumental variables methods, can be helpful in addressing further research questions posed by complex interventions. I intend to make the case that causal inference techniques have a lot to offer trials research in complex interventions.

Causation and Intervention

Accounts of causal discovery have traditionally split into approaches based on passive observational data and approaches based on experimental interventions that take control of (the distribution of) one or more variables. The former includes a vast number of techniques for the inference to causal structure on the basis of statistical features of data, while the latter provides in addition a me...

A Differential Approach to Causality in Staged Trees

In this paper, we apply a recently developed differential approach to inference in staged tree models to causal inference. Staged trees generalise modelling techniques established for Bayesian networks (BN). They have the advantage that they can depict highly nuanced structure impossible to express in a BN and also enable us to perform causal manipulations associated with very general types of ...

Causal inference from randomized trials in social epidemiology.

Social epidemiology is the study of relations between social factors and health status in populations. Although recent decades have witnessed a rapid development of this research program in scope and sophistication, causal inference has proven to be a persistent dilemma due to the natural assignment of exposure level based on unmeasured attributes of individuals, which may lead to substantial c...

Learning Optimal Interventions

Our goal is to identify beneficial interventions from observational data. We consider interventions that are narrowly focused (impacting few features) and may be tailored to each individual or globally enacted over a population. If the underlying relationship obeys an invariance condition, our approach can identify beneficial interventions directly from observational data, side-stepping causal ...

Understanding how pain education causes changes in pain and disability: protocol for a causal mediation analysis of the PREVENT trial.

INTRODUCTION Pain education is a complex intervention developed to help clinicians manage low back pain. Although complex interventions are usually evaluated by their effects on outcomes, such as pain or disability, most do not directly target these outcomes; instead, they target intermediate factors that are presumed to be associated with the outcomes. The mechanisms underlying treatment effec...
